Reading logs

Download a PDF and submit your completed log. You can read more about reading incentives on the Reading Incentive program page; instructions are also included on both versions of the form.

The fillable PDF file (‘Download log - fill’ button) will open in a new browser tab and can be filled out in most browsers and downloaded to digitally sign. PDFs are best filled out in Acrobat, PDF Expert, or similar applications; Preview is not recommended. The plain PDF (‘Download log - plain’ button) is ideal for printing and filling out by hand.

Submit completed logs via the submission form (‘Submit Current Log’ button) or via email to reading@westvincentpta.org.
